Who is Eligible?
· current GLYS member
· GLYS alumni 25 years of age or younger (on 1/31/08)

Criteria:
· must have completed 1 semester/equivalent of college, vocational or proprietary school
· must be enrolled during the semester scholarship is awarded (carrying 6+ hours)
· minimum grades for schooling completed: 2.0 (C) on 6+ hours or equivalent

   Postmarked no later than Thursday, January 31, 2008.
   If hand delivered, must be received by 6:00 p.m. on Thursday, January 31, 2008.

APPLICATION PACKET:

1.) Cover sheet with:
   a. contact information:
       · name
       · mailing address
       · social security number
       · telephone number and/or e~mail address
   b. basic criteria information:
       · copy of transcript/grade report that includes most recent semester/term
         2.0 (C) on 6+ hours or equivalent
       · proof of current enrollment (6+ hours)

2.) Please respond to these questions. Question responses should be on separate paper
(2 sheets maximum) and should NOT include your name or other identifying information.

Assume that the selection committee does not know you and is not familiar with your GLYS participation.
   a. Describe how you benefitted from participating in GLYS, and how GLYS and its members benefitted
       from your participation.
   b. Describe your personal growth that will help ensure your success in your education.
   c. As an adult, how do you see yourself contributing to the GLBT community in Buffalo or where
       you are living?
   d. How important would receiving this scholarship be to your education?
